If you have children ages 12-18, who wish to receive the vaccine from the MOE, you will need to fill up the attached Thai document (letter of consent for vaccines) and upload it to Google Form.
The English translation is for explanation purposes only.
Parents will also need to print the Thai version to submit at the time of the vaccine appointment.
